• The one time or monthly amount you wish to donate (please indicate which)
• Credit Card Number
• Expiry Date
• Name on the card
• Signature of card holder
• Email Address

       Your credit card will be charged the amount you have authorized and you will receive a receipt at year end from our Canadian office.

       You may also email this information to us at mail@hftnthailand.com in which case a signature will not be necessary.
